To get the most out of using Inciteful Med, you need to add your records to the tool.
You have a legal right to your medical records, so don’t be afraid to ask your doctor for them!
Finding Your Records
Most people can access their medical records through their patient portal: the online system your doctor or hospital uses to share information with you.
Look for platforms like:
- MyChart
- FollowMyHealth
- Cerner / Oracle Health
- Athena Health
- Allscripts / Veradigm
- Or a portal specific to your provider or health system
In your portal, you can usually find:
- Visit summaries
- Lab and test results
- Scans and imaging reports
- Doctor’s notes
You will be able to download your records as a PDF or plain text, which you can then add to Inciteful Med.
If you're unsure where to start, search your email for terms like “patient portal,” “MyChart,” or your provider’s name. You can also call your doctor's office and ask how to access your records online.

My Chart
If your doctor or hospital uses Epic for their electronic health record system (as most do in the U.S.), you’re likely familiar with MyChart, Epic’s patient portal. Your version may look a little different, with custom colors or logos to match your health system’s branding, but the layout and features are the same. Start by logging in.

You will then find your records in two primary places: the Visits and Test Results tabs.

Visits Tab
The Visits tab shows all your upcoming and past appointments. For each visit, you’ll find links to view your After Visit Summary and Clinical Notes. To save them, click “Print” in the top right corner and choose “Save as PDF.” You can also copy and paste the information directly into Inciteful Med.

Test Results Tab
The Test Results tab displays all your labs and scans in reverse chronological order. You can also search by test type or provider to compare specific test results over time. Click on any test to view and download it. To save a copy, either click “Print” in the top right and choose “Save as PDF,” or use the PDF download link often provided at the bottom of the report.
